Overview
Tirzepatide (LY3298176) is a synthetic single-chain peptide built on the GIP sequence and engineered to activate both the glucose-dependent insulinotropic polypeptide receptor (GIPR) and the glucagon-like peptide-1 receptor (GLP-1R). It is the archetypal “twincretin” and the most widely referenced dual incretin agonist in current receptor-pharmacology literature.

Tirzepatide is a 39-amino-acid peptide with α-aminoisobutyric acid (Aib) substitutions at positions 2 and 13 to resist DPP-4 cleavage, and a C20 fatty-diacid attached to the lysine at position 20 through a γ-glutamate and two 8-amino-3,6-dioxaoctanoic acid (OEG) spacers. The lipid moiety binds albumin, extending half-life in biological systems. The C-terminus is amidated.

Cellular structure & mechanism of action
GIPR and GLP-1R are class B GPCRs that signal through Gαs-mediated cAMP production. Pharmacological characterization shows Tirzepatide binds GIPR with affinity comparable to native GIP while binding GLP-1R with lower affinity than native GLP-1, and it displays biased signaling at GLP-1R, favoring cAMP generation over β-arrestin recruitment and receptor internalization. These properties make it a widely used tool compound for studying imbalanced and biased agonism at incretin receptors and for comparing dual-agonist signaling with selective GLP-1R agonists in islet, adipocyte and neuronal cell models.
